As a 501(c)3 entity, Faulkton Area Medical Center is required by law every three (3) years to conduct and display the results of the Community Health Needs Assessment (CHNA).
What is the improvement plan? The community health improvement plan (or CHIP) is a long-term effort to address public health problems based on the results of community health assessment activities and the community health improvement process. The assessment is created to address the public health problems based on the results conducted through the survey. A community health improvement plan should define the vision for the health of the community through a collective process and should address the array of strengths, weaknesses, challenges, and opportunities that exist in the community to improve the health status of that community.
Why do we ask you to complete the survey? The community health assessment gives organizations, like our own, information about the community’s current health status, needs, and issues. With this information, it can help develop a community health improvement plan by justifying how and where resources should be allocated to best meet community needs. It also helps other businesses in the community become aware of what they can do to help.
In years past, key takeaways from the CHNA have driven changes in the way that FAMC provides care and the different services added to the community of Faulkton, outside of FAMC; transit bus, chemotherapy/infusions, and more education to name a few. https://www.cdc.gov/publichealthgateway/cha/plan.html
